A 45-year-old obese male, ex-smoker with chest pain associated with 10 mm ST depression in multiple leads on the stress test. Cardiac catheterization showed 100% stenosis of the ostial left main and 70% stenosis in the mid-right coronary artery with preserved left ventricular systolic function. [Figure 1] and [Figure 2]. Patient subsequently underwent an uneventful multivessel coronary artery bypass surgery.
